If you provide the carrier with the CPT/HCPCS code, they can look them up for you to confirm coverage.
From experience, UR is normally a totally
separate department or in some cases an outside company that has contracted
with a health insurance carrier to provide UR support. Because of this,
you may notice a big disconnect between the Claims and UR departments. Meaning, the UR Department may issue an approval but the Claims Department may not have that approval on file which could result in an incorrect claim payment.
